The RTD drug test is directly observed, SAP-authorized, and must come back negative before you can drive again. Here is exactly how it works, what it tests for, and what happens if you fail.
You cannot order your own RTD test. The test must be employer-directed following written authorization from your SAP. If you take a drug test on your own before SAP clearance, it does not count as the RTD test and your prohibited status will not be lifted.
The DOT return-to-duty drug test looks identical to a regular DOT 5-panel urine test on paper — same panel, same SAMHSA-certified lab, same MRO review. The critical differences are:

Even if your original violation was for marijuana, the RTD test screens all five panels. A positive result on any panel — even a substance unrelated to your original violation — counts as a failure and resets the process.
Your SAP determines you have completed the prescribed education or treatment program and provides written authorization to your employer. Without this, no RTD test can take place.
Your employer (through their C/TPA or testing provider) selects a certified collection site capable of directly observed collections and sends you an authorization. The test is unannounced — you go immediately.
At the collection site, a same-sex observer accompanies you and watches the specimen leave your body into the collection cup. The collector documents the observed collection on the chain-of-custody form. You cannot refuse observation — refusal is treated as a positive test.
The specimen goes to a SAMHSA-certified laboratory. Negative results are reported within 24–72 hours. Non-negative specimens undergo GC/MS confirmation, and the MRO contacts you to review any legitimate medical explanation before reporting the final result.
The MRO reports the verified negative result to your employer and to the FMCSA Clearinghouse. Your prohibited status is lifted. Your employer can confirm this in the Clearinghouse before returning you to duty.
You return to safety-sensitive duties, but follow-up testing starts immediately. The SAP's plan — a minimum of 6 unannounced directly observed tests in 12 months — is now active in addition to your regular random pool testing.
A positive RTD test is a second violation — and it is reported to the FMCSA Clearinghouse immediately. You remain in prohibited status and cannot drive. The process resets:
There is no limit on attempts, but each failure makes the next prescription more intensive. SAPs are required to escalate treatment recommendations for repeated violations. A second positive RTD test typically results in inpatient treatment requirements rather than outpatient education.
If your original violation was an alcohol test result of 0.04 BAC or higher, the return-to-duty test is a breath alcohol test — not a urine drug test. The passing threshold is below 0.02 BAC. Direct observation applies to both types of RTD tests.
If you had both a drug violation and an alcohol violation, the SAP may authorize both an RTD drug test and an RTD alcohol test before clearing you to return to duty.
Yes, always. Direct observation is mandatory for all DOT return-to-duty drug tests under 49 CFR 40.67. A same-sex observer must watch the specimen leave your body. Refusal to be observed is treated as a refusal to test — which is the same as a positive result.
Your SAP authorizes it after you complete the prescribed education or treatment. Your employer then coordinates the test. You cannot self-order an RTD test — a self-ordered test does not lift your prohibited status.
You remain prohibited. The positive result is reported to the Clearinghouse as a second violation. You must return to your SAP for re-evaluation, complete a new (typically more intensive) prescription, and then undergo another directly observed RTD test.
Yes — once the MRO reports the negative result to the Clearinghouse and your status updates to "not prohibited," your employer can return you to safety-sensitive duties. The Clearinghouse update usually happens within 1–2 business days of the MRO result.
