How Long Is a DOT Medical Card Valid?
A DOT medical certificate (also called a Medical Examiner's Certificate, or MEC) is valid for up to 24 months from the date of examination. However, the examining physician may issue a shorter certificate if you have certain health conditions that require more frequent monitoring.
Shorter certificates are common
Many drivers receive a 12-month certificate rather than 24 months. If your blood pressure, vision, or another condition is borderline, the examiner may choose a shorter period. Always check the expiration date on your MEC — don't assume it's 2 years.
When Should You Renew?
Renew your DOT medical card before it expires — ideally 2 to 4 weeks before the expiration date. You can complete your renewal exam at any time while your current certificate is still valid.
Renewing early does NOT extend your current card
If you renew 30 days early, your new certificate starts from the date of the exam — not from your old card's expiration date. You do not lose any days. Renewing early is always the safe move.
What Happens If Your Card Expires?
If your DOT medical certificate expires, your CDL is automatically downgraded by your state DMV to a non-CDL license. You cannot legally operate a commercial motor vehicle in interstate commerce until you pass a new DOT physical and your state updates your record.
Your CDL itself does not expire — but without a current medical certificate on file with the FMCSA National Registry and your state DMV, the CDL has no operating authority. Getting reinstated requires passing a new DOT physical and waiting for the state to process the update, which can take 1–5 business days.
Can I drive while waiting for my new card?
No. Once expired, you cannot legally drive a CMV in interstate commerce — even if you just completed your renewal physical and are waiting for your state DMV to update the record. Keep your old card visible while it is still valid to avoid any gap.
How to Renew Your DOT Medical Card
The renewal process is identical to your original DOT physical — there is no separate "renewal" form. You simply complete a new FMCSA medical examination with a certified medical examiner.

Complete the FMCSA physical exam
Bring a government-issued photo ID, your glasses or contacts if you use them, your hearing aids if applicable, and a list of any current medications. The exam takes 30–45 minutes.
Receive your new Medical Examiner's Certificate
If you pass, the examiner issues your new MEC on the spot. Keep the paper copy — you must carry it while driving a CMV.
FMCSA National Registry is updated automatically
The examining physician reports your results to the FMCSA National Registry within 24 hours. Your state DMV typically updates your record within 1–5 business days.
What to Bring to Your Renewal Exam
There is no special preparation required for a renewal DOT physical. Bring the same items you would for any DOT physical:
- ✓ Government-issued photo ID (driver's license, passport, etc.)
- ✓ Eyeglasses or contact lenses if you use them for driving
- ✓ Hearing aids if you rely on them
- ✓ Current list of medications (prescription and over-the-counter)
- ✓ Any specialist clearance letters for conditions like sleep apnea, diabetes, or heart disease
- ✓ Your current DOT medical certificate (helpful but not required)
If you have a condition that previously required a specialist clearance, bring updated documentation — the examiner will need to review it before certifying you.
